What is Advance Care Planning?

ACP is the process of preparing for your healthcare and personal care needs if you are unable to communicate your wishes. It allows you to make decisions in advance about your care, giving peace of mind to both you and your loved ones.

Your ACP helps your medical team and chosen spokesperson make decisions that align with your best interests should you become too unwell to communicate. This process helps avoid guilt, stress, and potential conflicts among your loved ones when they need to make important decisions on your behalf.

Components of Advance Care Planning

Living Matters

ACP includes an understanding of what is important to you—such as relationships, lifestyle, religious or personal beliefs. This helps ensure that healthcare choices align with your personal values.

Medical Matters

You can decide in advance what treatments you wish to have or prefer not to have. This may include topics like pain relief options or instructions for situations such as if your heart were to suddenly stop.

Advance Care Planning (ACP) is valuable for everyone, regardless of age or health condition. It involves reflecting on your personal beliefs and values, sharing your healthcare preferences with loved ones and healthcare providers, and appointing a nominated healthcare spokesperson (NHS) to make decisions on your behalf if you’re unable to communicate.

This discussion results in an advance care plan that documents your wishes and serves as a guide for your healthcare team and loved ones in emergencies, helping ensure decisions align with your values and healthcare goals during critical times.

How to Complete
Your ACP

Note:
Your ACP does not dictate future medical decisions. If you lose the ability to make medical decisions, your healthcare team will consider your ACP and consult with your loved ones.

Reflect on What You Want

Think about the quality of life you want and the medical treatment you are comfortable with.

Choose Your Nominated Healthcare Spokesperson

Select someone you trust to share your care preferences. This person could be a family member or close friend. You may appoint up to two people as spokespeople.

Record Your Choices

Make an appointment with us to document your preferences in your ACP.

Review Your ACP

As life changes, you may make new decisions. When you do, update your ACP and inform your spokesperson.
