What is Advance Care Planning?

ACP is the process of planning for your
healthcare and personal care needs when
you are unable to communicate.

Your ACP helps your medical team & chosen spokesperson to make decisions that are in your best interest, Should you fall too unwell to communicate to make treatment decisions. 

ACP helps to avoid guilt, stress, and conflicts among your loved ones when making important care decisions for you.

ACP includes LIVING MATTERS to form an understanding of what is important to you (relationships, lifestyle, religious or personal beliefs). These will help determine healthcare
choice based on these personal value.

ACP includes MEDICAL MATTERS You can decide what treatments you wish to have or prefer not to have. Topics that may be discussed such as pain relief options or what to do should your heart suddenly stop.

Why is Advance Care Planning (ACP) important?

Advance Care Planning (ACP) is a valuable process for everyone, regardless of age or health condition.  It involves a voluntary discussion where you can express your preferences regarding your future health and personal care.

During the ACP discussion, you’ll reflect on your personal beliefs and values, sharing your healthcare preferences with both your loved ones and healthcare providers.  You’ll also have the opportunity to appoint a nominated healthcare spokesperson (NHS) who can make important decisions on your behalf if you’re unable to communicate.

This conversation results in the creation of an advance care plan, documenting your wishes.  This plan serves as a guide for your healthcare team and loved ones in making decisions during emergencies.  By having these discussions early on, your loved ones gain peace of mind, avoiding the need to make assumptions about your preferences during a medical crisis.  The advance care plan ensures that decisions align with your life values and healthcare goals, providing clarity during critical periods

How to do your ACP?

1. Reflect on what you want

Think about the quality of life you want and the medical
treatment you are comfortable with.

 

 

2. Choose your nominated healthcare spokesperson

The spokesperson is someone you trust to share your care preferences. He/she may be you family member or close friend.

You may appoint up to 2 persons to be your spokesperson.

3. Record your choices

Make an appointment with us to record your preferences on your ACP.

4. Review your ACP

As your life changes, you may make new decisions. When you do, update your ACP & your spokesperson.

Note: Your ACP does not dictate future medical decisions. If you lose capacity to make medical decisions in the future, your medical team will take your ACP into consideration and consult with your loved ones.
